I don't know if this has been caught yet, but I just got back from the D.C. consulate and they're using a form that is different than the one available on this site. This change may apply to the other U.S. embassies, but someone else gets to research that.

You can get the new application off their website however:

(I checked, it's the same one I had to use)

Also, they're asking that you not staple your pictures now, they want you to use a glue stick. Even though their web site says otherwise.

They also have a cover letter form I think they want you to use (you can't download it at this point in time). Although it's the same information as the cover letter they list on their website:

So... have a cover letter.

The application change is just a different format, I think all the information is exactly the same.

Side Note About D.C. Embassy:
For those of us who are familiar with the D.C. embassy, you have to walk through the first door now (you can't just follow the little path that goes directly to the visa door). If you do, then they'll make you go back out and through the first door (someone had to do that). Reason is they've got a metal detector now.

They're still doing an amazing job though. I didn't have any lines, I get my visa in 8 days. Everyone was very kind and friendly. The lady who was accepting my out-of-date application was very kind and explained everything to me; she even gave me the new forms to fill out there.

So even though I got a writers cramp filling out the new application, I have to say I wasn't disappointed at all with how easy they make the process.

Sorry, but for some reason when I tried to post the 2 not 5 links to the embassies web page, it wouldn't let me. Because today 2 = 5.

Anyway, just google: Russian Embassy (without quotes) and it's like the first or second result. Then just follow the directions to find the links.
